What Is Senior Care Coordination?
Senior care coordination is the proactive management and organization of all aspects of an elderly person’s care, including medical, social, and emotional needs. It is part of a collaborative process involving the client, their loved ones, and a senior care coordinator. This process includes thorough assessment, planning, and ongoing oversight to ensure the highest quality of life—and it doesn’t stop at home care.
At Everwell Homecare, we provide a comprehensive range of non-medical home care services for seniors and people with disabilities. We understand that clients’ needs often extend beyond basic care and may require specialized medical personnel such as nurses, physical therapists, occupational therapists, or psychotherapists. Seniors often face complex care requirements, including medical appointments, medication management, and emotional support. Family members supporting their care may struggle with communication across agencies, managing appointments (including transportation), and keeping up with paperwork, bills, and prescriptions.
Our dedicated senior care coordinators collaborate with you to ensure that all aspects of your loved one’s care are seamlessly integrated and customized to their specific requirements. With connections across Colorado and partner agencies nationwide, Everwell Homecare organizes care plans from a single point of contact for convenience and peace of mind.

How It Works
Setting up a care plan begins with filling out a referral form. A representative from Everwell Homecare will guide you through a step-by-step process:
What Does Senior Care Coordination Cost?
At Everwell Homecare Home Care, our senior care coordination services are fully covered by Medicaid. This includes coordination of home care, social support, and non-medical services. However, certain skilled medical services or services from partner agencies may not be fully covered by your state’s Medicaid program or may incur additional costs.
Our care coordinators work closely with all relevant state, federal, and partner agencies to streamline costs and make sure you receive every benefit you are eligible for. Our goal is to make care planning affordable, clear, and stress-free for you and your family.
